Direct Billing

Direct billing is available for many extended health plans. Confirm your plan and service with the clinic before your appointment.

Bring your insurer name, policy or group number, and member ID. Your insurer determines the amount covered and whether a referral is required.

The clinic can bill your primary plan where direct billing is supported. If you have a second plan, ask your insurer how to submit the remaining amount.

You are responsible for any fee your plan does not cover. Ask the clinic what you will need to pay at your visit.
